Central Bank of Jefferson City, Missouri is selected as the new Administrator of the Missouri Universal Service Fund. QSI Consulting, Inc. will be transitioning all reporting functions and records to Central Bank throughout the month of August. Central Bank will take over all administrative responsibilities effective September 1, 2007. Contact information for the new Administrator is as follows:

The Missouri Public Service Commission issued an order changing the payment date of Missouri USF reimbursements from a date no later than the last day of the month to between the 1st and 5th day of the following month. This change becomes effective April 29, 2007 and will apply to support payments made for the March 2007 and 1st Quarter 2007 reporting periods. Please note that requests for Missouri USF reimbursement shall be made to the Missouri USF administrator no later than the 15th of each month. The Missouri Public Service Commission approved an increase in the Missouri USF assessment rate applicable to all telecommunications services for which a bill is rendered in May 2007 or the first bill to customers issued after May 1, 2007. This order becomes effective March 25, 2007 and can be downloaded here.

The revised Assessment Determination and Remittance Form can be downloaded here. This form contains the revised assessment rate along with the contact phone number of the new MoUSF administrator. Please note that the data entry fields in this form can be populated electronically with the total MoUSF Surcharge Remittance on Line F calculated automatically.
